The purpose of the Housing Planning Coach is to provide competent, compassionate coaching to clients experiencing a housing crisis so that they resolve their housing crisis and prevent homelessness. The Housing Planning Coach is a team-oriented role that works collaboratively with Bucks County Housing Link staff.

Responsibilities

  • Maintain a housing-first perspective with a sense of urgency.
  • Answer phones and assist walk-ins with emergencies and utility assistance.
  • Assist clients in creating and working to meet goals to ensure housing stability.
  • Provide training to clients on how to identify appropriate housing opportunities.
  • Engage with and recruit landlords willing to work with individuals/families who are at risk of homelessness.
  • Assist clients in stabilizing their housing crises through mediation, problem-solving, and conflict resolution.
  • Administer financial assistance and/or provide referrals to appropriate community services.
  • Receive referrals and ensure that clients experiencing a housing crisis receive quality, compassionate services.
  • Empower clients to weigh and consider options, advocate for their own needs, and act on their own behalf.
  • Maintain complete, accurate, and timely case records, including HMIS data entry.
  • Meet regularly with the supervisor and Bucks County Housing Link staff to review client progress as well as to report on client outcomes.
  • Maintain positive working relationships with non-profit partners, social service agencies, funders, and local government offices.

Qualifications

  • Bachelor’s degree or equivalent combination of education and experience.
  • Computer proficiency – intermediate in HMIS, Microsoft Office, and database experience.
  • Exceptional flexibility, organizational skills, and the ability to prioritize tasks.
  • Valid PA driver’s license and auto insurance.

Salary: $40,000-$45,000

The purpose of this position is to inform and coordinate the referral and transportation needs of clients with mental health challenges. Primary duties will include: screening for eligible services, enhancing coordination of services, act as a liaison between the Lyft corporation and BCOC, collaborate with community partners, ensure utilization of transportation program, and accurately track data and outcomes of the transportation program.

This is a full-time position based out of Doylestown, PA.

Job Responsibilities:

  • Coordinate with providers to accept full and complete referrals, develop transportation plans, and schedule rides.
  • Ensure referring staff and client understand ride share requirements and process.
  • Monitor and track Lyft scheduled, completed, and missed rides in Excel, in real-time.
  • Minimize missed ride fees and ensure necessary, timely rescheduling is completed.
  • Ensure current and accurate data tracking on both Bucks County and BCOC internal Excel spreadsheets.
  • Maintain up-to-date, comprehensive knowledge about community & human services resources, with an emphasis on transportation resources in Bucks County.
  • Facilitate collaborative relationships between community resources, county partners and clients.
  • Provide direct assistance to potential clients to apply for appropriate community resources (Ex.  mental health services, BCT, etc).
  • Maintain log and provide screening information regarding CBCM STEPs eligible individuals to Supervisor for follow-up.
  • Demonstrate critical thinking and ability to provide continuous evaluation of CBCM Transportation program needs (increase referrals; provide suggestion for program improvement; data enhancement; client success, etc).

Requirements:

  • Bachelor’s degree in psychology or related field required
  • 2-3 years experience working in mental health field required.
  • Must exhibit exceptional professionalism, customer service and de-escalation skills.
  • Intermediate computer skills including intermediate proficiency in Excel reporting.
  • Valid driver’s license and reliable vehicle.

Salary range: $45,000-$50,000 per year
