Bucks County Opportunity Council (BCOC) is seeking an energetic and resourceful professional to be the new Rapid Rehousing Coach. This social service role assists low-income clients in Bucks County, with the highest level of barriers, to overcome obstacles, in order to achieve and maintain economic self-sufficiency. Responsibilities:
- Assist clients in stabilizing their housing crises through mediation and conflict resolution, administer financial assistance when appropriate, problem-solve and refer to related community services as appropriate.
- Support clients in developing and meeting goals to ensure housing stability.
- Maintain complete and accurate case records and client files, including HMIS and CTS data entry, while ensuring confidentiality. HMIS data must be entered within 5 business days.
- Foster client autonomy to evaluate options, advocate for their own needs, and act independently.
- Maintain positive working relationships with non-profit partners, social service agencies, funders, and local government offices.
- Complete follow up case management for 6 months post program exit
- Maintain a housing first perspective.
- Participate in annual training that relates to issues of homelessness, racism, poverty, TANF or related topic impacting clients.
Qualifications:
- Bachelor’s degree from four-year college/university; or one to two years related experience/training; or equivalent combination of education and experience.
- Exceptional communication skills, organizational skills and ability to collaborate effectively.
- Valid driver’s license and auto insurance.
Salary range: $41,000 – $45,000
The Reentry Stability Coach evaluates and manages referrals from the Bucks County Adult Probation and Parole and works with individuals discharging from the Bucks County Correctional Facility to establish housing stability plans. This role coaches clients in employment goals, to maintain housing and requires strong collaboration with the Housing Location team to assist clients in locating permanent, stable housing as expeditiously as possible. The Reentry Stability Coach partners with Family Service Association to coordinate treatment services for clients. This full-time position is based out of Doylestown, PA.
Responsibilities:
- Work collaboratively with Bucks County Adult Probation and Parole, the correctional facility, and Family Service staff to receive referrals, coordinate assessment, review treatment options, and housing resources to develop an individualized housing plan.
- Provide ongoing support for overall health and well-being that includes health care, stable housing, improvements in family life and personal relationships, community connections, education and vocational development, and employment.
- Assist clients in transition from correctional facility to temporary housing to permanent housing within 90 days.
- Develop an assistance plan that includes rental assistance for up to 12 months, including time in temporary housing. Make monthly payments to the landlord that include client contributions when applicable.
- Maintain complete and accurate case records, including CTS data entry, while ensuring confidentiality.
- Engage with and recruit landlords willing to give housing opportunities to individuals with criminal backgrounds.
- Represent BCOC at the Reentry Coalition for Bucks County, participate on committees, and be an advocate for reentry housing efforts in Bucks County.
- Meet regularly with your supervisor, Adult Probation and Parole, and Family Service to review client progress as well as to report on client outcomes.
- Maintain positive working relationships with non-profit partners, social service agencies, funders, and local government offices.
- Identify housing options, present available treatment and housing resources, assist with applications, and advocate on behalf of the client.
Requirements:
- Bachelor’s degree preferred from four-year college or university; or two to three years related experience and/or training; or equivalent combination of education and experience.
- 2-3 years’ experience working with the reentry population.
- Valid driver’s license, auto insurance, and use of a reliable vehicle.
